Background

Description of Commodities needed
As part of the implementation of one of our health programs in Cameroon, we invite qualified suppliers
to submit quotations for the supply and delivery of the infection, prevention and control commodities
listed below. And the pharmaceutical products listed in the second list below,

Submission of Quotations
Interested suppliers are requested to submit their quotations:

Required Documentation
Interested vendors must submit the following information as part of their quotation:
1. Administrative Documents
Suppliers must provide copies of the following company documents:
• Business Registration Certificate (Commercial Register)
• Up-to-Date tax Compliance Certificate
• Bank Account Details
2. Financial Proposal
The quotation must include Unit price and total price for each item (Proforma invoice)
3. Delivery and Payment Information
• Proposed delivery timeline and payment terms
4. Supporting documentation
• Any relevant technical or commercial documentation such as product specifications and
quality certifications where it applies.
Delivery location
Delivery location will be communicated to the selected vendor
Evaluation Criteria
Quotations will be evaluated based on the following criteria
• Compliance with the requested specifications
• Completeness and validity of administrative documents
• Price competitiveness
• Vendor experience in supplying similar products
• Compliance with legal requirements for the commercialization of the proposed products
FHI 360 Disclaimers
• FHI 360 may perform a background check on any selected vendor.
• FHI 360 may cancel the solicitation and not award
• FHI 360 may reject any or all responses received
• Issuance of the solicitation does not constitute an award commitment by FHI 360
• FHI 360 reserves the right to disqualify any offer based on failure of the offeror to follow
solicitation instructions
• FHI 360 will not compensate any offeror for responding to solicitation
• FHI 360 reserves the right to issue award based on initial evaluation of offers without further
discussion
• FHI 360 may choose to award only part of the activities in the solicitation, or issue multiple awards
based on the solicitation activities
• FHI 360 has the right to issue amendments to the RFQ at any time
