1. Background
FHI 360 invites qualified and eligible suppliers to submit quotations for the supply and delivery of
cartoons for the packaging, storage, and transportation of Infection Prevention and Control (IPC)
materials.
The purpose of this procurement is to identify a qualified supplier capable of delivering high-quality
cartons that meet the technical specifications, quality standards, and delivery requirements outlined
in this RFQ.
FHI 360 is a nonprofit human development organization dedicated to improving lives through
sustainable, locally driven solutions. Working in partnership with communities, governments, and
other stakeholders, FHI 360 advances health, education, economic development, research, and
humanitarian programs in countries around the world.
2. Scope of Work
The selected supplier shall supply and deliver cartoons according to the specifications below.

Technical Requirements
The cartons shall:
• Be suitable for storage and transportation of medical and IPC commodities.
• Be free from defects, tears, moisture damage, or deformation.
• Have adequate strength to withstand stacking and transportation.
• Be new and unused.

3. Vendor Eligibility Requirements
Interested suppliers shall submit the following:
Mandatory Administrative Documents
• Business Registration Certificate
• Taxpayer Certificate / Tax Compliance Certificate
• Valid Tax Identification Number (TIN)
• Bank Account Details
4. Evaluation and Award Criteria
Quotations will be evaluated using the following criteria:

5. Basis for Determination of Fair and Reasonable Pricing
The evaluation committee may determine fair and reasonable pricing using one or more of the
following methods:
1. Price competition where two or more responsive quotations are received and compared.
2. Comparison with historical prices paid for similar purchases.
3. Comparison with current market prices.
4. Comparison with published catalog or commercial pricing, when available.
6. Representations and Certifications
By submitting a quotation, the supplier certifies that:
• The information provided is accurate and complete.
• The supplier is legally authorized to conduct business.
• The supplier complies with all applicable laws and regulations.
• The supplier has no conflict of interest relating to this procurement.
7. FHI 360 Disclaimers
• FHI 360 may perform a background check on any selected vendor.
• FHI 360 may cancel the solicitation and not award
• FHI 360 may reject any or all responses received
• Issuance of the solicitation does not constitute an award commitment by FHI 360

• FHI 360 reserves the right to disqualify any offer based on failure of the offeror to follow
solicitation instructions
• FHI 360 will not compensate any offeror for responding to solicitation
• FHI 360 reserves the right to issue award based on initial evaluation of offers without further
discussion
• FHI 360 may choose to award only part of the activities in the solicitation, or issue multiple
awards based on the solicitation activities
• FHI 360 has the right to issue amendments to the RFQ at any time
